РЕЗЮМЕ
В статье представлены современные данные о наиболее значимых осложнениях длительной терапии глюкокортикоидами (ГК), включая метаболические нарушения, стероид-индуцированный сахарный диабет, артериальную гипертензию, сердечно-сосудистые осложнения, ГК-индуцированный остеопороз, поражение кожи и вторичную надпочечниковую недостаточность. Особое внимание уделено современным международным рекомендациям по рациональному применению ГК, основанным на принципах glucocorticoid stewardship, предусматривающих использование минимальной эффективной дозы в течение максимально короткого периода времени, регулярный мониторинг безопасности терапии и своевременную профилактику осложнений. Представленные клинические наблюдения демонстрируют тяжелые кожные проявления хронической ГК токсичности и подчеркивают необходимость раннего выявления нежелательных явлений, индивидуализации лечения и мультидисциплинарного подхода к ведению пациентов.
